This will be all the more certain if there are multiple factors that point in this direction.

This combination corresponds with abnormal functioning of the nervous system, and is often coupled with hard aspects from Uranus, Jupiter, or Mars in the charts of people who have migraines, traumatic brain injuries, or some other issue that adversely impacts the head. Since Pluto corresponds with abnormal growth, this combination can also signify abnormal growth within the brain or elsewhere within the nervous system. Ebertin lists “nervous irritation” as a biological correspondence of Mercury/Pluto. Whereas Mercury/Neptune may correspond with memory loss as a result of an illness or medication, Mercury/Pluto corresponds with memory loss as a result of psychologically traumatic life experiences. Mercury/Pluto is one of only two Mercury combinations that denote fertility issues. The other such combination is Mercury/Saturn. Mercury/Pluto can also correspond with intestinal trauma, especially if Mercury is in Virgo.
This combination denotes a mind that may struggle to distinguish between fact and fiction. These people may live in a fantasy world of their own design, and may be able to inspire others to join them there. They are creative wordsmiths and fantastic storytellers.
